Before the visit, list every medication and supplement you take, with doses and how often, and note any you have stopped or missed. Track any side effects and how well each medication is working.
Note how many refills you have left and which pharmacy you use. Write down questions, like whether any medication could be simplified or is still needed.
Expect your doctor to review your list, check how things are working, and update your prescriptions. Lead with your full list so nothing is missed.
Take notes as the doctor talks, record the meeting (with their permission!) in myStoria, or add a quick voice note to myStoria as soon as possible afterward so that you are not relying on memory alone.
