An ADHD visit runs on examples, not labels. Concrete notes about how attention, focus, and organization play out in your day give your doctor far more to work with.
Jot down where focus, memory, or restlessness get in the way, at work, at home, and in relationships, with real examples. Note when it started and whether it shows up in more than one part of your life.
List anything that affects attention, like sleep, stress, caffeine, or other conditions, plus current medications. If you have old report cards, past assessments, or notes from people who know you well, bring them.
Expect questions about your history, your daily patterns, and how long this has been going on. Share your examples early so the visit moves toward a plan rather than staying general.
Take notes as the doctor talks, record the meeting (with their permission!) in myStoria, or add a quick voice note to myStoria as soon as possible afterward so that you are not relying on memory alone.
Before you leave the clinic, ask for copies of anything from the visit, like lab results, assessments, or referrals, and upload them to myStoria to keep your history complete. (If you aren't able to get copies, connect with Concierge and we can help you source them!)
If you start a new approach or medication, keep a short daily note on focus, sleep, and mood so you can tell what is actually changing. Bring it to the next visit.
If something feels off, like side effects or no change after the agreed time, book a follow-up rather than waiting. Small adjustments are normal early on.
