COPD, or chronic obstructive pulmonary disease, is managed over time, and your own tracking is part of that. Clear notes on your breathing and flare-ups help each visit build on the last.

— Your appointment, start to finish

Three steps to a COPD visit that keeps you steady

01
Before your visit

Prepare

Track your breathing day to day, how far you can walk before getting winded, and how often you use a rescue inhaler. Note any flare-ups, changes in mucus, and nights when sleep was disrupted.

Bring your full medication and inhaler list with how you use each one, plus any recent infections or hospital visits. Note your smoking history and any exposures at home or work.

Bring these questions

  • Is my current treatment keeping me stable?
  • Am I using my inhalers correctly?
  • What is my plan when a flare-up starts?
  • Would pulmonary rehab or vaccines help me?
  • Which symptoms mean I should get care right away?
02
During your visit

Make the time count

Expect your doctor to review your symptoms, check your breathing, and go over your inhaler technique and results from any lung function tests. Share your notes so nothing about your flare-ups is missed.

Take notes as the doctor talks, record the meeting (with their permission!) in myStoria, or add a quick voice note to myStoria as soon as possible afterward so that you are not relying on memory alone.

Before you leave the clinic, ask for copies of anything from the visit, like lab results, assessments, or referrals, and upload them to myStoria to keep your history complete. (If you aren't able to get copies, connect with Concierge and we can help you source them!)

03
After your visit

Follow up

Stick with your daily medications and keep tracking your breathing and rescue-inhaler use. A written action plan helps you catch a flare-up early and know what to do.

If you are more breathless than usual, your mucus changes color or amount, or a rescue inhaler is not helping, follow your action plan and seek care promptly.

How myStoria helps

Every appointment starts prepared

myStoria keeps your symptom history, medications, and questions together, so you walk into each appointment ready and pick up cleanly at the next one. When you want guidance between visits, you can schedule a video consult in advance with a member of our team, several of whom are nurses.
