A follow-up visit is where you and your doctor check how things are going since last time, so coming in with what has changed makes the time count. Here is what to prepare, what to ask, and how to keep things on track.

Three steps to a follow-up that moves things forward

Before your visit

Prepare

Before the visit, note what has changed since your last appointment, like symptoms, side effects, or how a treatment is working. Track anything you were asked to watch, and bring any readings or results.

Write down any new questions and list your current medications, including any changes. If tests were ordered last time, have the results or know where they are.

Bring these questions

  • How am I doing compared to last visit?
  • Should we adjust my treatment or medications?
  • Do I need any more tests or referrals?
  • When should the next check-in be?

During your visit

Make the time count

Expect your doctor to review your progress, go over any results, and adjust the plan. Lead with what has changed so the visit focuses on next steps.

Take notes as the doctor talks, record the meeting (with their permission!) in myStoria, or add a quick voice note to myStoria as soon as possible afterward so that you are not relying on memory alone.

Before you leave the clinic, ask for copies of anything from the visit, like lab results, assessments, or referrals, and upload them to myStoria to keep your history complete. (If you aren't able to get copies, connect with Concierge and we can help you source them!)

After your visit

Follow up

After the visit, note any changes to your plan and put the next appointment or test in your calendar. Keep tracking whatever you were asked to watch.

If something changes or gets worse before your next visit, contact your provider rather than waiting for the scheduled date.
