Three steps to a blood pressure visit that keeps you on track

Before your visit

Prepare

Track your home readings with the date and time, taken the same way each time, so your doctor sees your real range. Note any symptoms like headaches or dizziness, and any readings that stood out.

Write down your current medications, doses, and whether you have missed any, along with any side effects. List changes in your diet, activity, salt, alcohol, or stress since your last visit.

Bring these questions

  • Is my blood pressure where it should be?
  • Should we adjust my medication or doses?
  • Which lifestyle changes would help most?
  • How often should I check at home, and when should I call?

During your visit

Make the time count

Expect your doctor to check your pressure, review your home readings, and talk through medication and lifestyle. Lead with your readings so the visit focuses on adjustments, not guesswork.

Take notes as the doctor talks, record the meeting (with their permission!) in myStoria, or add a quick voice note to myStoria as soon as possible afterward so that you are not relying on memory alone.

Before you leave the clinic, ask for copies of anything from the visit, like lab results, assessments, or referrals, and upload them to myStoria to keep your history complete. (If you aren't able to get copies, connect with Concierge and we can help you source them!)

After your visit

Follow up

After the visit, keep taking your readings on the schedule you agreed, and note any side effects from changes. A steady record shows whether the plan is working.

If you get a very high reading with chest pain, breathlessness, a severe headache, or vision changes, seek urgent care. Otherwise, book your follow-up with your readings in hand.
