Nobody sat you down and explained what a hot flash is. You got the word, maybe a joke about fanning yourself at a dinner party, and that was the whole briefing.
So here's what got skipped: a hot flash isn't your body overheating. It's your brain misreading the temperature and reacting fast. Once you see how, the sweating, the flush, the 2 a.m. wake-ups, and what to do about them all start to make sense.
Fun fact: your core temperature barely moves during a hot flash. The heat you feel isn't heat you made, it's heat your body is shedding on command, because your brain decided you were too warm.
That decision happens in the hypothalamus, the part of your brain that runs body temperature, along with hunger, sleep, and hormones. Think of it as a thermostat. In perimenopause, that thermostat gets touchy.
Your body tolerates a small range of internal temperature without doing anything about it. Scientists call it the thermoneutral zone. Drift up a little, you don't sweat. Drift down a little, you don't shiver. There's slack built in.
As estrogen, specifically estradiol, drops through perimenopause, that slack disappears. The zone narrows to almost nothing. A tiny rise in core temperature, the kind you'd never have noticed at 35, now trips the "too hot" alarm.
Why estrogen? Deep in the hypothalamus sits a cluster of neurons nicknamed KNDy, after the three chemicals they run on: kisspeptin, neurokinin B, and dynorphin. Estrogen normally keeps them calm. When estrogen falls, KNDy neurons grow larger and fire harder, and they push on the brain's heat-loss center. This is the leading model, and a strong one: drugs that block the neurokinin B signal reduce hot flashes, which is hard to explain unless this pathway is driving them. Some details are still being worked out, where exactly each temperature threshold sits is debated, but the core wiring holds up.
Once your narrowed comfort zone hits its ceiling, the hypothalamus triggers an emergency cool-down.
