Diabetes care is built on patterns over time, so a visit that draws on your readings and habits does far more than one starting from a single number. The record you bring makes the plan sharper. Here is what to prepare, what to ask, and how to follow up.

Three steps to a diabetes visit that keeps you on track

Before your visit

Prepare

Track your blood sugar readings with dates and times if you monitor at home, and note any highs, lows, or patterns you have spotted. Bring your most recent A1C if you have it.

Write down your current medications and doses, whether you have missed any, and any side effects. List changes in your diet, activity, weight, or stress, and note how your feet, vision, and energy have been.

Bring these questions

  • Are my blood sugar and A1C where they should be?
  • Should we adjust my medication or doses?
  • Which diet and activity changes would help most?
  • Which checks do I need, like eyes, feet, and kidneys?

During your visit

Make the time count

Expect your doctor to review your readings, check on medications, and talk through diet, activity, and the regular checks that go with diabetes. Lead with your numbers so the visit focuses on adjustments.

Take notes as the doctor talks, record the meeting (with their permission!) in myStoria, or add a quick voice note to myStoria as soon as possible afterward so that you are not relying on memory alone.

Before you leave the clinic, ask for copies of anything from the visit, like lab results, assessments, or referrals, and upload them to myStoria to keep your history complete. (If you aren't able to get copies, connect with Concierge and we can help you source them!)

After your visit

Follow up

After the visit, keep monitoring on the schedule you agreed and note any side effects or patterns. A steady record shows whether the plan is working.

If you have very high or very low readings, confusion, a fruity smell on your breath, or a wound that will not heal, seek care rather than waiting. Otherwise, book your follow-up with your readings in hand.
